Definition

Primary Lesions  

Macule

Nonpalpable lesion < 1 cm in diameter

Patch

Nonpalpable lesion > 1 cm in diameter

Papule

Palpable lesion < 1 cm in diameter

Plaque

Palpable lesion > 1cm in diameter

Pustule

Lesion containing purulent fluid

Vesicle

Lesion < 1 cm that contains clear fluid

Bulla

Lesion > 1 cm that contains clear fluid

Nodule

Firm papule or plaque that is deeper in the skin

Secondary Lesion  
Erosion Superficial defect to level of epidermis or superficial dermis (may be linear "excoriations" or "annular"
Ulcer Defect extends to dermis or deeper
Lichenification Thickening of skin suggestive or chronic rubbing or scratching
Verruca Papillomatous or warty changes
Purpura Purple nonblanching lesions caused by hemorrhage into the skin. May be pinpoint "petechiae"
